Thank you for your interest in supporting Young Survival Coalition (YSC) by becoming a “YSC Champion” through a third-party event or initiative. We appreciate your willingness to raise money and awareness for our organization and the fight against breast cancer. Events and promotions sponsored by YSC Champions provide much-needed funding for our programs and services.

YSC is extremely grateful to the many individuals and organizations who organize events to help support us. As you might expect, it’s important that every third-party event meets specific criteria and directly benefits YSC. We review each event on a case-by-case basis before determining whether to approve it. Have questions? Please review our Event-Based Fundraising FAQs.


An individual or organization who is interested in planning an event to benefit YSC must submit this completed and signed form at least fifteen (15) days prior to the event and agree to adhere to YSC’s guidelines relating to YSC Champions events. Only those events that support our mission and legitimately benefit YSC will be approved. This is an important safeguard to preserve the integrity and reputation of YSC, as well as our commitment to our constituents and the community.

YSC and the Better Business Bureau request that the percentage of proceeds that will be donated are communicated to the public in clear, unambiguous language and in a readily identifiable fashion on all communication materials (e.g. "20% of net proceeds" or "$10 from every ticket sold"). Please give us an idea by filling out ticket price in $USD and Amount/Ticket Donated in $USD or a percentage. Additionally, please estimate the overall donation amount for the event below.
Please give us some details on your proposed community event. Include potential sponsors if applicable and how you intend to publicize and/or promote the fundraiser.

Applicant affirms by submitting this application that they have read and agree to to the YSC Champions Event Guidelines. YSC is not liable to any party or vendor for any fees, costs, or payments of any kind. Applicant agrees to indemnify and hold harmless YSC against any claims by third parties or vendors for such fees, costs, or payments incurred pursuant to this agreement. The parties in this agreement are not joint ventures, partners nor representatives of each other and such parties have no legal relationship.
